Please ensure your contact information is up to date. We request you update them regularly to ensure we are able
to contact you for your results, appointments and messages.
Please limit your visit to 1-2 of your most concerning issues, and book follow-up visits for any remaining concerns.
Same Day Appointments
For our regular patients we will try our best to arrange for same day appointments for urgent issues.
There are also a limited number of same day walk-in appointments.
Walk-in visits are to be limited to one problem only.
No narcotic or controlled medications like Tylenol # 3,4, sleeping medication, sedatives, antianxiety medications etc. are prescribed to Walk-In patients.
Test results are not discussed over the telephone. It is requested to kindly book an
appointment to review your test results.
Notification of the referral appointment by telephone or mail can take up to 3 weeks. If you do not hear from
the clinic or the specialist directly within this time frame, please call the clinic for an update.
Please book an appointment or come as Walk-In if your medication is running low.
Narcotic (Triplicate) or controlled medications like Tylenol # 3, 4, sleeping medication, sedatives,
anti-anxiety medications etc. prescriptions cannot be given for longer than 1 month to our regular patients.
These consultations are not offered. Please book an appointment if you would like to speak to a physician.
Not all services are covered by Alberta Health and Wellness.
The staff should be able to confirm the charge before you see the doctor.
You will be asked to pay for uninsured services prior to receiving them.
We accept both Cash, Debit cards and most of the Credit Cards.
If a patient is rude and/or aggressive they will be requested to leave the clinic immediately and the appointment/walk-in visit will be cancelled.
In worst cases, 911 will be called and the person responsible will be reported to Police
We respect your confidentiality. You must provide us with written, witnessed consent form in order to release your medical information to anyone (even a parent, a counselor or a spouse), unless you are with the person at the time of disclosure.
Forest lane medical clinic is proud to host University of Calgary's Faculty of Medicine Medical Students and Resident doctors at this clinic.
Medical students are trained to take medical histories and perform certain physical examinations. The supervising doctor makes the diagnosis and decides on management with the medical student.
Resident doctors see patients with less direct supervision (as they have graduated medical school). A resident doctor can diagnose and treat your condition, and will have a supervisor readily available.
The supervising doctor is responsible to ensure that your problems are accurately addressed. You may be asked to see a Medical Student or Resident Doctor and we encourage you to allow them to facilitate us with your care.
If you do not want to see a student or resident doctor, please inform the staff or the student/resident and we
accommodate your request for that visit.
Termination of Doctor-Patient Relationship
For best possible health care for the patient, a strong doctor-patient relationship is needed.
When this relationship becomes compromised it may be terminated. If this occurs, we will provide
emergency medical care for one month or when the patient finds a new family doctor.
If our services are required outside the normal hours you will be requested to dial 811 (Health Link).
